Office of School Counseling
Please complete the forms and contact us at James River during the hours of 7:00AM until 3:00PM Monday through Friday to set up an appointment for enrollment for the 2008-2009 school term. We will begin accepting appointments after spring break on Monday, March 31. My number at JRHS is 378-2427. Be certain you have gathered all items on the enrollment checklist and bring them with you. I am a notary public.
Only a parent or legal guardian may enroll a student.

Records for facilitating initial enrollment:
| Proof of Residence: “Deed of Trust” OR a lease in your name and the “Proof of Residency” Form AAA-1418 or Form 1418A AND Form 1418B if you are living with someone else. The forms must be notarized. |
| Original birth certificate (with raised seal) - required for any first-time Virginia student or parent must complete affidavit. |
| Immunization Record (including three Hepatitis B) signed by a physician, RN or Health Department official. |
| Special Education information (IEP and evaluations: educational, psychological, social history report). |
| Unofficial school records (report cards, testing records, etc.). |
| Expulsion Affidavit this form must be notarized. |
Must be received to complete enrollment:
| Official school transcript, including withdrawal grades, testing, information |
| Social Security card |
